Homeowners 3 (HO-3) Special Form

Coverage A – Dwelling
(Dwelling coverage is based on the calculated replacement value of the home.)
Size of home, quality or grade of materials used to construct, cost of labor, # of room or bathrooms, size of garage(s), pools, screened enclosures, lanais, patios, porches, etc. are all factors in establishing your homes valuation.

Coverage B – Other Structures
(normally 1%, 2%, 5%, and 10% of the Dwelling Limit)

Coverage C – Personal Property Coverage
(normally 10% – 50% of the Dwelling Limit)

Coverage D – “Loss of Use”
(normally 1%, 2%, 5%, and 10% of the Dwelling Limit)

Coverage E – Personal Liability Coverage
(normally $100,000 – $500,000, but some companies may allow an increase to $1,000,000)

Coverage F – Medical Payments
(normally $1,000, $2,000, $3,000, $5,000 or $10,000)

All Other Perils Deductible
(normally $500, $1,000, $2,500, $5,000 or $10,000)

Hurricane Deductible
(normally $1,000, or 2%, 5%, 10%, 15% of the Dwelling Limit)

Sinkhole Deductible
(normally $1,000, $2,500, $5,000, or 10% of the Dwelling Limit)

Homeowners 5 (HO-5) Comprehensive Form

Coverage’s are similar to but often much broader than the HO-3 Form

  • Provides “open perils” coverage for the Dwelling, Other Structures, and Personal Property and is the broadest coverage form of any of the homeowners forms
  • Normally only available for High Value Dwelling programs ($500,000 or greater in Dwelling coverage or 4,000 sq. ft. in size or larger)

What Homeowners 3 and 5 Insurance Forms Normally Cover?

Examples include but are not limited to:

  • Accidental discharge of water (pipe break)
  • Fire or Lightning
  • Smoke
  • Windstorm or Hail
  • Freezing of plumbing system
  • Theft or Vandalism
  • Catastrophic Ground Collapse
  • Sinkhole (exclusions may apply depending on carrier and geographic area)
  • Explosion
  • Falling objects or aircraft (including spacecraft, missiles, or vehicles)
  • Riot or civil commotion

Homeowners 8 (HO-8) Modified Coverage Form

Most coverage’s are similar to the HO-3 Form

  • Provides “limited perils” coverage for the Dwelling, Other Structures, and Personal Property and is the most basic coverage form of any of the homeowners forms
  • Normally excludes water damage coverage’s (i.e. pipe breaks, roof leaks, etc.
  • Normally limits Theft coverage
  • Normally excludes Sinkhole coverage

  • Earth Movement (Earthquake), Settlement
  • Inherent Vice, Latent Defect, Defect or Mechanical Breakdown
  • Intentional Loss
  • Professional services provided or not provided
  • Neglect
  • Ordinance or Law (unless added by endorsement)
  • Ownership, operation, use, or maintenance of aircraft, motor vehicles, and certain watercraft
  • Animal Liability exposure (limitations may be applicable)
  • Sinkhole
  • Trampoline Liability exposure
  • Water and Flood Damage
